In short
Four free logs, each on one printed page and as a spreadsheet (CSV): the New York LCSW, the New York LMHC, the New Jersey LCSW, and California's AMFT weekly log. California's Board requires a weekly log and publishes its own form; ours copies its fields. New York and New Jersey publish no log, so ours for them are suggested logs, built from what each state's forms ask your supervisor to certify.
Licensure Hours tracks New York LCSW, LMHC and LMFT, New Jersey LCSW and California LMFT hours today.
On this page9 sections
New York LCSW
Your supervisor keeps the record NYSED may ask for: “the client contact hours in diagnosis, psychotherapy and assessment-based treatment planning and supervision hours” (8 NYCRR 74.6). Form 4B tells you to use “the psychotherapy log”, completed by you and your supervisor and kept by your supervisor, but the form has no log page and we found no official one. Ours follows what Form 4B asks your supervisor to certify.

New York LMHC
“The supervisor is responsible for maintaining documentation of the applicant's supervised practice and hours of supervision” (8 NYCRR 79-9.3). Form 4B reports the dates and the total hours practicing mental health counseling. Ours splits them into direct hours, other indirect hours and supervision.

New Jersey LCSW
The Board's Proposed Plan asks your supervisor: “Do you agree to maintain weekly supervision notes and co-sign a client contact log which shall be made available to the Board upon request?” It publishes no log, so ours uses the three sections of the Documentation form (A, B and C) and the weekly supervision rule.

California AMFT
“Supervisees shall maintain a weekly log of all hours of experience gained toward licensure. The log of hours shall be signed by the supervisor on a weekly basis” (16 CCR 1833(d); 1869(d) for ASWs and 1820(d) for APCCs). The Board publishes its own weekly logs. Ours copies the fields of the MFT log, 37A-525; where the Board requires its own form, use the official one.

Does New York require a weekly log?
For the LCSW, NYSED's forms page says “The psychotherapy log should be completed weekly to record client contact hours and supervision hours” and that the completed log “should be retained by the qualified supervisor”. NYSED publishes no template. Writing your hours down each week, and having your supervisor initial them, keeps the record straight.
Printing and the spreadsheet
Each log prints on one landscape page; print as many as you need. The CSV opens in Excel, Google Sheets or Numbers, with the same columns. Both are plain blank forms: nothing is filled in on our site, and nothing you write is sent anywhere.
