In short

A printable copy and a spreadsheet (CSV) of the fields on the Board's MFT Weekly Log of Experience Hours, 37A-525 (Revised 08/2022): one row for each week, columns A to C, and your supervisor's signature. It is not the Board's form; where the Board requires its own form, use the official 37A-525. The Board requires a weekly log, signed by your supervisor every week, which you keep and send only if asked.

What the Board requires of the log

“Supervisees shall maintain a weekly log of all hours of experience gained toward licensure. The log of hours shall be signed by the supervisor on a weekly basis.” It includes the name and address of your work setting, your hours in each category for each week, and your totals for each week and each category (16 CCR 1833(d)).

The columns, as on 37A-525

  • A. Direct counseling with individuals, groups, couples or families
  • A1. Diagnosis and treatment of couples, families and children: part of A
  • B. Non-clinical experience: supervision, psychological testing, writing clinical reports, writing progress or process notes, client-centered advocacy, and workshops, seminars, training sessions or conferences
  • B1. Supervision, individual or triadic, and B2. Supervision, group: parts of B
  • C. Total hours per week: A + B. Don't add A1, B1 or B2 again
  • Your supervisor's signature, every week

One log per supervisor and setting

The form: “Use a separate log for each supervisor and for each work setting.” Keep hours before and after your degree on separate logs too (the Board's FAQ).

Do I send the weekly log to the Board?

No, not unless the Board asks: “Do not submit to the Board unless specifically requested.” You keep the signed logs until you are licensed. When you apply, each supervisor's Experience Verification form carries your hours; the logs can't replace it.

The BBS Experience Verification form: who signs it, when, and what it asks

ASW and APCC logs

The Board publishes its own weekly logs for ASWs and APCCs, with their own columns (the ASW log, for example, has a line for clinical psychosocial diagnosis, assessment and treatment and caps a week at 40 hours). This copy is the MFT log only; use the Board's log for your registration.
