Up to 24 months, plus up to two 12-month extensions at NYSED's discretion: 48 months at most. Ask for an extension at least 60 days before the permit expires.
The rule
“The limited permit in mental health counseling shall be valid for a period of not more than 24 months, provided that the limited permit may be extended for no more than two additional 12-month periods at the discretion of the department” (8 NYCRR 79-9.4).
You may not practice until the limited permit is issued, even after graduating.
Extending it
- Submit a new Form 5 and the $70 fee, with a justification for the extension
- NYSED asks for it at least 60 days before the permit expires
- Extensions are at NYSED's discretion, not automatic
Leaving a job: the clock can stop
When you leave, your employer must return the permit to the State Board office with an explanation. NYSED voids it and, in its words, the “clock will stop”. The remaining time is available for a future permit. Ask your employer to return it promptly.
Changing setting or supervisor
Your permit names the setting and supervisor. NYSED's FAQ says there is no extra fee to add a setting or change employer or supervisor, but you must submit a new Form 5 with the updated information; its forms page also lists a Limited Permit Change Form. Check the current forms page, and send a Form 5CS for each new supervisor.
When the time runs out
If you have not met the requirements when the permit and its extensions expire, you must stop practicing, except in an exempt setting.

- The expiry date printed on your permit, and the date 60 days before it
- Every date a permit was returned, so you know how much time is left
- Which setting and supervisor each permit names
