NY diagnostic privilege forms 1D, 2D, 4D and 5D, and checking your status

In short

You file Form 1D online with the $175 fee. Each school sends Form 2D with a transcript, and your supervisor, or a licensed colleague who knows your work, sends Form 4D straight to NYSED. Form 5D, with your supervisor's Form 5CS-D, is the $70 limited permit for gaining privilege hours. NYSED gives no status by phone: allow six weeks after everything is in, then use its Contact Us form.

Licensure Hours tracks New York LCSW, LMHC and LMFT, New Jersey LCSW and California LMFT hours today.

On this page10 sections

Form 1D: the application

  • Online only, with a $175 non-refundable fee: eservices.nysed.gov/professions/wf/mhc-1d for LMHCs, eservices.nysed.gov/professions/wf/mft-1d for LMFTs
  • You must already be licensed in New York in that profession. It asks for your six-digit license number and the date your current registration ends (on NYSED's verification site, your license or your registration certificate)
  • Seeking the privilege in two professions takes two applications and two fees
  • You can come back to an unfinished application within 30 days, and add documents later, with your Application ID and date of birth

Form 2D: your education

  • Only for the privilege's education: “Education Taken to Meet the Continuing Education Requirement of Your Original Mental Health Practitioner License WILL NOT BE ACCEPTED”
  • You complete Section I; the registrar completes Section II and sends all pages of the form (it is three pages long), with an updated official transcript, directly to NYSED. NYSED won't accept it from you
  • One from each program that gave you credit toward it
  • Why again, when you sent transcripts for your license: NYSED says those documents “did not provide the specific information required” for the privilege

Form 4D: your experience

  • One form for LMHCs, LMFTs and psychoanalysts; you check your profession and complete Section I
  • Licensed on or after June 24, 2024: the 2,000 hours. Licensed before: the 2,000 hours or three years (36 months) of experience, with the three-year route due by June 24, 2027
  • Your direct supervisor completes Parts A and B; if they aren't available, a licensed colleague with first-hand knowledge of your supervised experience completes Parts A and C
  • It goes to NYSED from them, never from you: “This form will not be accepted if submitted by the applicant”
  • NYSED's forms page speaks of “both pages”, but the form is three pages long; send all of them

Who can sign Form 4D?

A supervisor in a profession allowed for your privilege: an LCSW; a psychologist educated and trained in psychotherapy (the regulation's words; NYSED's privilege page says only a licensed psychologist); a psychiatrist certified by the ABPN; and, from June 24, 2024, an LMFT with the privilege for an LMFT, or an LMHC with the privilege for an LMHC.

Form 5D: the diagnostic limited permit

  • For a licensed LMHC or LMFT gaining the 2,000 hours: “This limited permit authorizes supervised practice while gaining experience to meet the 2000 direct client contact hours required for the diagnostic privilege”
  • Online, $70, non-refundable: eservices.nysed.gov/professions/wf/mhc-5d or .../mft-5d
  • With Form 5CS-D: you complete Section I, your New York supervisor completes Section II before you submit it, and you make sure it is complete
  • Valid two years, renewable twice for a year at a time ($70 each), four years in all; a supervisor may have at most five permit holders of any kind
  • A new supervisor or setting: the online Limited Permit Change Form (eservices.nysed.gov/professions/wf/limited-permit-change), with a supervisor certification for each new supervisor, and no fee
  • NYSED says the permit was extended to June 24, 2027 with the supervised-setting rule; the law's expiry clause names only the setting rule. Ask NYSED before relying on a permit after that date

How do I check the status of my application?

NYSED: “We cannot provide the status of a licensure application by phone. After you have submitted all your documentation, please allow 6 weeks before submitting a Contact Us Form to request a status update. Contacting us earlier may extend processing times for applications.” NYSED publishes no processing time for the privilege.

When it is granted, NYSED emails you, and the privilege appears on your record on its verification site; there is no new certificate unless you ask for one.

Keeping it

The privilege is renewed with each three-year registration: the registration fee, $45 for continuing education and $175 for the privilege. NYSED's privilege page gives the registration fee as $196 in one place and $193 in another; its fee chart says $196. Your renewal notice shows the fee charged.
