- The week, Monday to Sunday
- Direct hours: your contact with clients
- Indirect hours: recordkeeping, case management, research, supervision and professional development
- Supervision hours, individual or group, in person or by video
- Your supervisor’s initials, so the two of you agree week by week
What it adds up to
3,000 hours in all, at least 1,500 of them direct, and at least half direct in every setting. Keep one page per supervisor and setting: direct hours in one setting cannot be combined with indirect hours in another. Every NY LMHC requirement.
Why the extra boxes
- Permit dates: your permit lasts up to 24 months; ask for an extension at least 60 days before it expires.
- Your supervisor’s license and privilege: hours under a supervisor qualified for the diagnostic privilege may later count toward it. What NYSED said.
- The setting and a contact that lasts: years later, these are the details people cannot find.
