In short

A free weekly sheet for your supervised clinical experience toward the New Jersey LCSW, on one printed page, as a four-year PDF log with running totals, or as a spreadsheet. It is a suggested log, not an official form: the Board publishes none. Its columns are the three sections of the Board's Documentation form, A, B and C, with your weekly supervision and a place for your supervisor to co-sign.

On this page7 sections

A suggested log, not an official form. The New Jersey Board publishes no log form. These columns follow sections A, B and C of its Documentation of Supervised Clinical Experience form (Rev. 4/11/25) and the weekly supervision rule (N.J.A.C. 13:44G-8.1(b)).

Total

One log per supervisor, job title and location, as the Board asks for Documentation forms.

Client sessions by video or phone: we haven't found a Board answer on whether they count as face-to-face client contact, so this log keeps them out of A, B and the total, in their own columns. The supervision columns are part of C, not added to the total again.

The Proposed Plan asks your supervisor to keep weekly supervision notes and co-sign a client contact log, available to the Board on request.

What to write each week

  • A. Face-to-face clinical client contact: clinical assessment and clinical consultation. The form accepts up to 960 of these toward your 1,920
  • B. Psychotherapeutic counseling: at least 960 hours are required
  • C. Non-clinical hours, including your time in supervision: at most 1,080 count
  • The week's total, A + B + C, as the form adds them
  • Client sessions by video or by phone, in their own columns and not in A, B or the total: we haven't found a Board answer on whether they count as face-to-face client contact
  • Your supervision that week, part of C: individual face to face, group face to face, and individual by video
  • Your supervisor's co-signature

New Jersey's Documentation of Supervised Clinical Experience form, explained

The weekly supervision rule

“At least one hour of face-to-face individual or group clinical supervision per week”, or one hour of individual supervision by HIPAA-compliant live video, “for no more than half of the total supervision hours” (N.J.A.C. 13:44G-8.1(b)1.v). The video column lets you keep an eye on the half.

Why a co-signed log

The Proposed Plan asks your supervisor to agree “to maintain weekly supervision notes and co-sign a client contact log which shall be made available to the Board upon request”. The Board doesn't say what the log must look like. When you apply, the Documentation form asks for your hours by section, your total months (at least 24), your dates and whether the months were consecutive; weekly totals make those easy.

One log per supervisor, title and location

As the Board asks for Documentation forms (one per supervisor and title on the form, and one per location in its FAQ): start a new log when your supervisor, your job title or your location changes.
