Keep your own records
NYSED makes the supervisor responsible for documenting the supervised practice and hours of supervision. No copy of a counselor's log is emailed to you, so keep your own.
When your supervisee is ready to apply, you complete Section II of Form 4B (your license, the setting, the dates and their total clock hours), sign it and send it to NYSED yourself. NYSED won't accept it from them.

Do I need an account?
No. Each batch comes as a link by email; it opens in your browser, with nothing to install and no password.
Does it cost me anything?
No, and it never will. Your supervisee pays for their own record; reviewing it is always free for you.
What if a week looks wrong?
Open the week and ask for a correction. Your supervisee fixes it and sends it back; nothing is confirmed until you say so.

Does this replace Form 4B?
No. Form 4B is NYSED's form: you complete Section II from the weeks you confirmed, sign it and send it to NYSED yourself.
